```Duties:```
- Provide patient care by assessing and monitoring patients' medication therapy, identifying drug-related problems, and recommending appropriate interventions.
- Collaborate with healthcare providers to develop and implement patient care plans.
- Utilize clinical knowledge and expertise to optimize medication therapy outcomes.
- Conduct medication reconciliation and review medication orders for appropriateness.
- Ensure compliance with regulatory requirements and standards of practice.
- Participate in interdisciplinary rounds and provide drug information to healthcare team members.
- Perform aseptic technique for preparation of sterile products, including IV infusions.
- Provide education to patients and caregivers on medication use, side effects, and precautions.
```Experience:```
- Doctor of Pharmacy (Pharm.
D.
) degree from an accredited college or university.
- Current state licensure as a pharmacist.
- Minimum of 2 years of experience in a clinical pharmacy setting, preferably in acute or critical care.
- Strong knowledge of utilization management principles and medical management practices.
- Proficiency in pain management strategies and protocols.
Note: Experience with utilization review is a plus.
