Under the general direction of the director of nursing, the purpose of this position is to assist with the management and direction of resident care standards, resident care and supervision, and direction of all direct care providers and provide assistance with supervision and management for the nursing department.
As an MDS (Minimum Data Set) Coordinator, you will play a crucial role in ensuring accurate and comprehensive documentation and assessment of patient needs in a healthcare facility.
Your primary responsibility will be to complete and submit MDS assessments according to regulatory guidelines.
*EDUCATION:* * Graduate of an accredited school of Registered Nursing or Licensed Vocational Nursing and currently licensed in the State of Texas * 2 years’ experience as a nurse (preferred).

*Quality Assurance/Performance Improvement Responsibilities:* * Receive direction from the owner/board of directors on QAPI goals and clearly communicate these to staff.

Communicate effectively with patients and their families, explaining the purpose and benefits of the assessment process.
- Compliance: Stay updated with federal, state, and local regulations related to MDS assessments.
Ensure compliance with all applicable regulations, guidelines, and protocols to maintain accuracy and integrity in the assessment process.
- Quality Improvement: Participate in quality improvement initiatives, ensuring continual evaluation and enhancement of MDS assessment processes.
Collaborate with the quality improvement team to implement evidence-based practices and improve patient outcomes.
- Training and Education: Stay current with changes in MDS regulations, software, and processes.
Provide training and education to staff members on MDS assessment protocols, best practices, and documentation requirements.

Make routine inspections of the facility to assure that established policies and procedures are being implemented and followed.

Participate in state/federal surveys of the facility.
13.
Development of plans of correction for cited deficiencies.
Ensure such plans incorporate timetables and methods of monitoring to ensure that such deficiencies do not recur.
14.
Provide governing board with information relative to survey findings, plans of correction, monitoring guidelines, and follow-up survey results.
15.
Delegate a responsible staff member to act in your behalf when you are absent from the facility.
Ensure that all facility personnel, residents, visitors, etc.

Act as liaison between the facility and regulatory agencies, fiscal intermediaries and resident advocacy agencies.

Assume accountability for all data contained in the employees’ handbook.

Assume responsibility for identification, investigation and follow up on concerns identified in the facility Quality Indicator report.

Ensure that an initial resident assessment is completed within fourteen days of the resident’s admission.

Ensure that a comprehensive care plan is completed within twenty-one days of the resident’s admission.

Assist the nursing staff in encouraging the resident and his/her family to participate in the development and review of the resident’s plan of care.
